Role Summary

As a Remote Clinical Care Coordinator, you will deliver essential value-based care services to U.S. patients from the comfort of your home in the Philippines. Working closely with U.S.-based physicians, care teams, and healthcare organizations, you will be responsible for the day-to-day clinical execution of Remote Patient Monitoring (RPM), Chronic Care Management (CCM), Collaborative Care Model (CoCM), and Principal Illness Navigation (PIN) programs.

This is a clinical service delivery role—not billing or administrative work. You will directly engage with patients, monitor health data, coordinate care plans, and help drive better health outcomes for individuals managing chronic and complex conditions. Key Responsibilities

  • Monitor patient vital signs and health data through RPM platforms, identifying trends, red flags, and opportunities for early clinical intervention.
  • Conduct scheduled and as-needed patient outreach calls for CCM services, including comprehensive care plan development, medication reconciliation, and ongoing care coordination.
  • Support CoCM programs by collaborating with psychiatric consultants, primary care providers, and behavioral health specialists to manage patients with behavioral health conditions.
  • Deliver PIN services for patients choosing the right providers
  • Document all patient interactions, clinical assessments, and care coordination activities accurately and in compliance with CMS guidelines.
  • Track and log billable time for each service line (RPM, CCM, CoCM, PIN) to support accurate clinical documentation and program compliance.
  • Communicate with U.S.-based care teams to escalate clinical concerns, share patient updates, and ensure continuity of care.
  • Stay current with CMS program requirements, clinical best practices, and any regulatory changes affecting RPM, CCM, CoCM, and PIN services.
  • Participate in quality improvement initiatives and team huddles to continuously enhance patient outcomes and program performance.

Qualifications

Required:

  • Based in the Philippines with a reliable high-speed internet connection and a dedicated, quiet home workspace.
  • Active and valid healthcare license in one of the following: Registered Nurse (RN), Licensed Practical Nurse (LPN), Advanced Practice Registered Nurse (APRN), Certified Medical Assistant (CMA), or Behavioral Health Care Manager (BHCM).
  • Minimum of 3 years of hands-on experience delivering RPM, CCM, CoCM, and/or PIN services for U.S.-based healthcare providers.
  • Strong working knowledge of U.S. Medicare and Medicaid billing codes relevant to these programs, including CPT codes 99453–99458 (RPM), 99490/99491 (CCM), 99492–99494 (CoCM), and more
  • Proficiency with electronic health record (EHR) systems and remote patient monitoring platforms.
  • Excellent English communication skills, both written and verbal, with the ability to build rapport with U.S.-based patients over the phone.
  • Strong clinical assessment, critical thinking, and care coordination skills.
  • Willingness to work U.S. business hours (schedule may vary by client time zone).

Preferred:

  • Experience working in a remote or telehealth setting for U.S. healthcare organizations.
  • Familiarity with care management platforms such as ChronicCareIQ, Prevounce, Signallamp Health, or similar tools.
  • Background in chronic disease management for conditions such as diabetes, hypertension, COPD, heart failure, or behavioral health disorders.
  • Previous experience in a BPO or outsourced healthcare services environment.
